These are two different questions that's why they can be used in one sentence in a form of interrogation.

I've never heard about this rule from the English grammar despite the fact I've been an English language tutor for many years. Since my secondary school's graduation I keep on translating & interpreting the British & the Americans.
I'll be very grateful to you, VIP Foxman, for your reference as for that English grammar rule's origin.
